Here is the link to Executive Order 2025-16 that applies to all OHCA Providers. This must be completed by 5pm, Nov 21, 2025.

Per OHCA, failure to complete, sign and return the attestation by 5 p.m., Nov. 21, 2025, will be considered non-compliance with the Executive Order and could result in payment suspension, contract denial and/or contract termination.

"Recipient does not have eligibility for this transaction date"

DMH seems to be having an issue with eligibility in their system. You can resubmit the CDC once DMH has fixed their problem. You will highligh the CDCUW and click on submit.
